Cervical Cerclage

A cervical cerclage may be recommended if you have

  • A history of second-trimester pregnancy loss
  • A short cervix detected on ultrasound
  • Previous preterm birth
  • Cervical surgery such as LLETZ or cone biopsy
  • Signs of cervical weakening in the current pregnancy

The cervix normally remains long and closed during pregnancy, but in some women, it may begin to shorten or open too early, increasing the risk of preterm labour. A cerclage helps strengthen the cervix by placing a stitch around it, keeping it closed until it is safe for the baby to be born.

Diagnosis is made through a transvaginal cervical length scan, which accurately measures cervical length. If the cervix measures below a certain threshold, especially before 24 weeks, a cerclage may be advised.

There are two main types of cerclage

  • History-indicated cerclage performed early in pregnancy based on previous obstetric history.
  • Ultrasound-indicated cerclage performed when a short cervix is detected during pregnancy.

The procedure is usually carried out under regional anaesthetic and takes around 20–30 minutes. Women typically go home the same day or the next, depending on individual circumstances.

After the procedure, close follow-up is important. Dr Subramanian provides ongoing monitoring with

The cerclage is normally removed at around 36–37 weeks of pregnancy, unless early labour begins sooner.
